我院自2011年年5月开展出院患者电话回访以来，共回访患者23200人次，其中健康指导15623人次，提供健康咨询14320人次，指导复查4380人次，解决患者疑难问题510人次，化解患者意见32次。对回访过程中患者反馈的意见和建议均在回访登记本上做详细的记录，并认真梳理，综合分析后，及时反馈给有关部门、科室或个人，对医务人员起到了监督作用。

1具体做法

电话回访的形式和主要内容:医院成立“回访中心”，选派具有20多年临床工作经验的护士专职承担该工作。通过院内微机中心直接获取患者的资料，专职随机抽取前三天出院患者的10%，通过电话调查了解患者对治疗、护理及后勤工作的意见及建议。同时解答患者提出的相关问题，并将回访信息详细填写在专用的登记本上，于每月、每季度、每半年、每一年进行总结，及时上报有关部门，采取相应措施。通话内容包括四个方面：①跟踪患者的愈后情况，掌握患者的动态信息，及时给予关心和帮助。②征求患者及家属对医疗、护理及后勤服务的意见及建议。③了解患者满意的医生、护士。④通话结束，向患者对医院工作的支持表示感谢。

电话回访应注意的事项

语言亲切，态度诚恳，注意语音语调，必要时融入笑容。遇到不懂得地方语言的患者或家属时，要用标准的普通话交流，要温文尔雅，让对方感受到你的诚意。

正确把握医疗、护理安全尺度，电话回访的目的是给患者带去医院的问候，跟踪患者的康复情况，了解满意度，不是远程医疗。回答患者医疗问题时需要谨慎，不能简单的判断和随意指导，更不能在电话中给患者确诊。

回访时应遵循一定的规则，电话接通前要详细了解患者的基本信息，包括患者的姓名、*别、年龄、出院时间、出院诊断以及疾病的转归等，使回访有的放矢。电话接通后，首先问对方一声“好”，然后自报家门，再确定对方身份，并说明致电的目的，取得对方合作。根据不同疾病询问患者的康复情况，给予适当的健康指导。患者提出的问题能回答的马上给予答复，如术后如何进行功能锻炼、婴儿喂养过程中的种种情况、专家坐诊时间等。不能马上回答的问题咨询专家后再给予答复。若电话号码错误，接听电话者与患者素不相识，做好解释工作的同时，要礼貌的说一声“对不起,打扰你了”，然后将电话慢慢放下。当得知患者

已经死亡时，要特别注意语言、语气及说话的分寸。应理解患者家属悲痛的心情。

表示你的同情和遗憾，并告知如有需要我们帮助的地方，请与我们联系。切忌一听到患者死亡，认为没有必要再征求意见，立刻挂断电话，这样容易使患者家属产生不满意和疑虑，发生事端[1]。当个别患者对医疗、护理工作不满意而宣泄，甚至脏话出口时，回访人员切忌同样恶语相向，应耐心倾听，因为认真倾听对方的诉说是对患者和家属极大地尊重，并且倾听可以最大程度的缓解冲突[2]。在倾听的同时，寻找机会给予引导和解释，最终取得患者的理解。有寻求帮助的患者，要做到尽力帮助，如做不到，要同患者解释清楚，不能随便承诺，否则容易失信于人，影响医院声誉。对出院患者进行满意度调查，了解患者住院过程中有没有遇到不愉快的事情，并征求患者意见及建议，将调查结果准确记录。通话结束，对患者或家属的配合表示感谢，等对方挂机后再放下电话，以示对对方的尊重。

2结果

2017年回访患者4280例，满意4112人，满意率；点名表扬医务人员423人次，表扬护理单元132次，其中星级病房72次，占；不满意168人，占；其中医疗不满意110人，护理不满意68人。

3讨论

医疗问题分析及对策。

费用高:从收集归类的意见可以看出，医疗费用高占据医疗问题的第一位。分析其原因：①检查、用*超出了医保目录范围，致费用不报销或报销比例低。②社会保险、医疗保险等保险制度尚不够完善，许多费用及贵重*品需要个人自费。③过度用*使*品比例增高，患者负担加重。

采取的对策：①掌握社会保险、医疗保险及新农合的各项政策及规定，争取目录内用*和检查，提高费用报销比例。②建议进一步完善各种医疗保险制度，尽量扩大目录用*范围，使参保人员得到更多的实惠。③诊疗过程中，不过度检查和用*，控制*品比例，降低医疗费用。医生虽然不能左右各种费用的价格，但可以通过合理检查、合理用*、降低住院天数等措施来为患者省钱。比如用低价*能达到效果的，就不用高价*，不做与疾病无关的检查等等，立足为患者提供质优价廉的治疗方案和服务，从而满足患者少花钱，看好病的愿望。④医院管理者加大监督检查力度，定期和不定期抽查出院病历检查合理用*情况，特别是抗生素的合理应用，做到奖惩分明。对医务人员进行职业道德教育，提高职业道德修养，树立一切为患者着想，全心全意为患者排忧解难的道德观念。

一、国内访问学者通过学习和工作，接触和了解本学科的学术前沿动态，加深基础理论，拓宽知识面，提高学术水平，为回原单位后发挥学术带头人或学术骨干的作用打下基础。

二、国内访问学者必须具备坚实的基础理论和专业知识，有较强的独立从事教学和科研工作的能力，并具有一定的实践经验，一般应是副教授。

三、接受国内访问学者的单位应有博士导师或学术造诣较深的教授担任指导教师，科研工作开展较好，在实验设备、图书资料等方面有较好的条件，一般应是有博士学位授予权的学科点或国家投资重点建设的实验室。

访问学者工作总结

在省教育厅的支持下，本人于20XX年8月，有幸前往英国牛津大学工程科学系进行学习和研究。目前作为访问学者的学习生活已经圆满完成，回顾半年来的访问经历，牛津大学的浓厚的科研氛围、严谨的教学态度教学让我深受感染，在科研和教学工作方面获益匪浅。我从以下几个方面进行说明：

一、科学研究方面

本人在牛津大学工程科学系时，研究对象为分布式环境下的医疗决策支持系统的设计与实现。我们结合Fox教授所创建的用于描述医疗决策过程的Proforma语言和Tallis engine等工具，结合英国的国家健康服务的现状，开发了一种基于多代理的，在分布式环境下能够使用的医疗决策支持系统的原型系统，并结合国内医疗的情况，对该系统进行了一定的改进。

二、教学实践方面

作为全世界最古老的大学之一，牛津大学有着悠久的教学传统，本人在牛津大学期间，通过旁听本科生及研究生课程，深刻地感觉到教学效果的提升，不但来源书本知识的传授，更来自于师生对学科的认识与感悟。所以在教学过程中，应该在以课程学习为主的`同时，坚持以实践学习为重。

在出国留学申请过程中，学校通常会要求2～4封来自申请者工作主管或者教授的推荐信函。推荐信是申请文书中非常重要的组成部分。因为推荐信是申请材料中唯一的一个从第三者的角度对申请人学习成绩、学术科研能力、研究潜质和性格特点的评价。

所以，推荐信的份量直接关系到能否出国留学，能否获得奖学金等。

推荐信的内容

推荐信一般应包括下列内容：

1.被推荐人的基本情况介绍。侧重于个人的毕业时间、学校、所获学位以及个人的专业经历。

2.推荐人对被推荐人的基本评价。侧重于被推荐人的专业基础、个性、特点、工作态度和在学术上的前途估计。若是推荐研究生，推荐人还需进一步说明其深造学习的基础和当前所具备的研究能力。显然，恰如其分地评价被推荐人的基础、能力和前途，比言过其实的赞誉更令人情服，更具有实际意义。

3.推荐人可以着重介绍被推荐人曾经获得的奖励，发表过的论文，参加过的重要学术会议，以及曾在学生组织或学术团体中的任职等来支持自己的评价。

4.推荐人还必须清楚地表明被推荐人留学的身份是研究生还是访问学者，专业领域和研究方向是什么。

如果大学提供了现成的推荐表格，则必须按这类表格认真逐项填写。推荐表格中一般有学生综合评估的一项，即要求推荐人说明该生在所教的学生中应列为前5%，10%，或25%等。这种评估是指教授的个人评价，可以稍高一点。

另外，推荐表格上常有许多难以填写的项目，遇到该情形可填“I don't know”，亦不致对该生的评价有何严重影响。

如何写推荐信

推荐信是留学申请过程中，最难由自己掌握的部分，也由于他是众多申请材料中少数能从第三者的可观角度评关申请者的.文件，因此有重要的地位。我们可以从三个方向着手：

1.就学校方面

首先，必须先了解申请专业的特点(推荐信内容可以强调此特点);整理各校对推荐信内容的要求，即可得到关于评估的基本要求(例如，学术研究能力、创造力等)，以及特殊要求(例如，对研究领域的适应度等)。试想：你认为录取委员会希望由推荐信获取那些关于你的信息。

2.就个人方面

分析自己的优缺点，在校成绩以及工作表现，有无特殊经历可以展现自己在此研究领域的潜力，或拥有那些特殊的能力,依此作为选择老师写推荐信及提供了解你的基础。并以明确具体的证据来支持证明自己的特殊能力和个人特点(例如，试验成果、研究计划、学术出版物、课外活动等)。

3.就推荐者方面

推荐信是了解申请人的重要依据，在研究生课程中尤其如此。申请人可以选择现在或以前的老师、教授或雇主。推荐人必须熟悉学生的学习情况，了解学生的学习目的是否明确，在学术上是否有前途，以及学生的适应能力、创造能力、品行和特长。

书写推荐信的最理想的人选是一封由系主任写，一封由专业课教授写，一封由自己的导师写。不过，对方若无规定，则除了自己的亲属之外，任何人皆可为推荐人。

推荐人一般应具备高级技术职称，如教授、副教授、研究员、副研究员等。如果推荐人在国内外学术界享有盛誉，或者被申请就读学校聘为客座教授，或者曾经在该校讲过学，那么这样的推荐人写的推荐信就具有很强的效力了。

注意事项

一般而言，推荐信需要三封，最好三封各有其强调的重点，以完全申请人的特点与才华，推荐信的基本组成除信头、发信日期、收信人性命、地址、称呼、签名、推荐人性命、职务等外，信件内容还应包括以下各项：
